WELLNESS AFTERCARE SERVICE (The Oasis)

The Premium Wellness Aftercare Program is a 24/7 online/telephone counselling support system that provides psychological and wellness support for both patient and families during treatment and after discharge from all rehabilitation and treatment centres. The program is designed to provide a constant support system to re-integrate them back to their environment and remain well.


Wellness Aftercare program package includes:

• 24/7 telephone and online counselling
• Monthly reminders and sessions follow up
• Life style modification therapy
• Telephone support
• Monthly bio psychosocial tips SMS blast
• Crisis intervention and lifestyle coaching
• Physical/Virtual Group Session



STRUCTURE OF THE AFTERCARE SERVICE:

The Premium Wellness Aftercare Service would run in two phases:
1. DURING ADMISSION (INCARE)
2. AFTER DISCHARGE (AFTERCARE)


DURING ADMISSION PHASE:

At this phase, the wellness team is involved in the treatment process with the patient even before discharge to encourage rapport building as well as the sense of calling in distress or when necessary. Services offered during admission includes;

• Wellness group sessions
• 24 hr Telephone counselling services


DISCHARGE PHASE

During the discharge phase, the aftercare support is continued for another period of 9 months as is an annual subscription. The modalities include;

• Bi-weekly general follow up calls
• Outpatient / Session reminder calls to patients to see their clinician
• A bio psychosocial tip monthly sms
• Bi- monthly family calls
